Cancellation Form

This form is intended for all students who wish to CANCEL their membership with Karate Mission Viejo / Novastar Martial Arts, or any associated studios.

By filling out this form you AGREE AND UNDERSTAND:

1) You ARE CANCELLING your membership contract with our STUDIO(S).  That means your current spot in class will be relinquished in 30 days time.

2) Per your contract with the STUDIO, you AGREE to discontinue your training in 30 days from the date of this form, you AGREE to pay your last 4 weeks of training (last month), and any outstanding balance to your account. NO EXCEPTIONS.  You can request a statement of account at any time by emailing sensei@karatemv.com.

3) If you have any make up lessons available you may still use them during these thirty days.  You can trade any remaining lessons for personal sessions if it’s easier.  2 group sessions = 1 personal session.

4) If you choose not to show up to class, and/or use your outstanding make up lessons (if any), you understand ALL of these will be lost after your membership STOPS.

5) You will ONLY be charged for your last 30 days of training.  Once the last payment has cleared all your information will be archived, and your payment information deleted.

6) If your last payment is not received in full, your account will remain ACTIVE and this request to cancel will be DENIED.  You understand you will be responsible to continue to pay for your program until you make your last payment.

    7) You are allowed to keep all your training “EQUIPMENT” (including uniform(s), sparring gear, training weapons, etc.), and you are welcome to re-start your training at any time. However, if prices change after your re-start, you will be training under an updated program.

    8) If you choose to donate any EQUIPMENT to the studio please reach out to 949-712-9150.  If you leave any EQUIPMENT at the dojo after the STOP date, you are indicating all of this (these) is being donated to STUDIO, and you agree to relinquish any ownership to it.  We will reach out to you ahead of time in case we find any equipment left.

    Please let us know if you would like to donate any items you will not use in the future, sparring gear, uniforms, training weapons, etc.
    If you are willing to leave a good review of our program online we are willing to give you a two-week credit on your last payment (so you will only pay the last 2 weeks)
