Time Off / Injury Hold Form

This form is intended for Active students who need to hold their program due to extended absence or due to an injury/illness.

By filling out this form you understand:

1) You are NOT cancelling your contract with STUDIO.  If you do not know when you will be back, you must fill out a Cancellation form instead.

2) Your spot in class will be HELD for as long as you request it. When the hold expires your program will continue AUTOMATICALLY, whether you show up or not.  Remember – you are buying a spot in class.

3) All of these requests will be processed as soon as possible and will be effective after 2 WEEKS.  This means you will still pay for 2 weeks before the hold beigns.  If you pay monthly, your amount will be prorated accordingly (YOUR MONTHLY TUITION divided by 2)

    You must select how long you expect to be out. During this time you will NOT be charged but as soon as the time lapses your program WILL REACTIVATE AUTOMATICALLY regardless of whether you show up to class or not.
    You must select a return date. This is a private school and we must have at the very least an approximation. If you need to update this later you can contact us via text message.
